Student Policies and Enrollment Agreement

Please read each section carefully before signing. Your checked box at the end of this document confirms that you have read, understand, and agree to all of the policies below.

1. Classroom Conduct and Dress Code

Dress code (required for every class, no exceptions):

Scrubs, or long pants with a plain t-shirt

Socks and closed-toe shoes

Not permitted: skirts, dresses, or shorts

Cell phones and recording: Cell phones, cameras, and audio or video recording of any kind are not permitted in the building. Phones must be stored and powered off during class. If you have an emergency situation that requires you to be reachable, notify your instructor before class begins.

Behavior: Disruptive or disrespectful behavior toward instructors, staff, students, or clinical partners will result in immediate dismissal from the program with no refund of any funds paid.

2. CNA Eligibility

The CNA prep course is designed for Florida certification only.

If you have failed the CNA exam three times, the Florida Board of Nursing requires you to complete a 120-hour training program before testing again.

The State conducts an FBI background check on all applicants. If you have any prior arrests or convictions, it is your responsibility to verify your testing and licensing eligibility with the Florida Department of Health before enrolling.

You agree that all fees are non-refundable if you are found ineligible to test or become certified by the State of Florida.

3. Tuition, Deposits, and Fees

ALL FEES ARE NON-REFUNDABLE. This includes deposits, registration fees, tuition, and all state and national testing fees.

If you are unable to attend your scheduled class, you may receive one courtesy transfer to a future class, provided you notify MTPB before the class start date. Transfers requested after the class has begun will not be granted.

No refunds will be issued under any circumstances.

4. Physical Ability and BLS/CPR Course Notice

You confirm that you are physically able to perform the tasks required by the program in which you are enrolling.

American Heart Association notice for BLS courses: If you have reservations about your ability to perform CPR or First Aid on an injured or cardiac arrest victim, you should consider this before enrolling. The AHA has developed instructional materials for this purpose. Use of these materials in an educational course does not represent course sponsorship by the AHA, and any fees charged for such a course do not represent income to the Association.

5. Phlebotomy Course Consent

If you enroll in the Phlebotomy program, you acknowledge and agree that:

You consent to having venipunctures performed on you by instructors and other students, and to performing venipunctures on other students.

MTPB is not responsible for any injury caused by you to another participant, or caused to you by another participant.

MTPB does not guarantee a specific number of successful draws. Employers may have their own draw requirements that exceed what is completed in this course.

6. EKG Course Notice

EKG classes require some exposure of skin for proper placement of electrode pads. Female students should wear a sports bra or bikini top underneath their clothing when attending this class.

7. Agreement

I have read and understand all of the policies above. I agree to abide by all terms of this agreement. I understand that any violation may result in my removal from all MTPB programs with no refund of any funds paid.
